The Santa Fe Prevention Alliance, in partnership with the Santa Fe County DWI Program, the Santa Fe Community College and the New Mexico Office of Substance Abuse Prevention, invites you to participate in a survey of Santa Fe City and Santa Fe County residents. Survey findings will inform and evaluate programs in our community that aim to prevent and decrease underage drinking and problematic prescription drug use in our County.

Participation in this survey is voluntary. This survey should take you approximately 10 minutes to complete. No identifying information about you will be associated with your responses on this survey. Your responses will remain completely anonymous. Upon completion of the survey, you have the option of participating in the drawing for a free $20 gas card. You will be taken to a new site and the information provided will not be connected with your responses on this survey.
