Please take a minute to participate in the Annual Santa Fe County Community Health Survey!
The Santa Fe Prevention Alliance, in partnership with the Office of Substance Abuse Prevention, Santa Fe Public Schools, and the Santa Fe County DWI Program invites you to participate in a Community Health Survey of Santa Fe County residents. Survey findings will inform and evaluate programs that aim to decrease, prevent, treat and reduce the harm of problematic alcohol and drug use in our community.
Participation in this survey is voluntary. This survey should take you approximately 15 minutes to complete. No identifying information about you will be associated with your responses on this survey. Your responses will remain completely anonymous.
