Santa Fe County Invites Public to Health and Healthcare Town Hall Meeting

You are invited to a Town Hall Meeting focusing on Children and Families and their experiences of health and health care in Santa Fe County.

WHEN: Tuesday, February 28, 5:00 pm to 7:00 pm

WHERE: Santa Fe Public Library Southside Branch, 6559 Jaguar Drive, Santa Fe, NM 87507

Santa Fe County Community Services Department is creating an Accountable Health Community and conducting a gap analysis to identify challenges and barriers within the County’s health and social service systems. The Accountable Health Community model addresses unmet non-medical and social needs of residents, by linking clinical care and community services to improve community health, increase quality of care, and reduce health care costs.

“We are committed to building a system that helps people navigate community services and offers providers access to data and information to better serve residents, improve health, and reduce health care costs to the system overall,” explains Rachel O’Connor, Director of Santa Fe Community Services Department.

Co-sponsors of the Town Hall Meeting are CHRISTUS St. Vincent, the City of Santa Fe, SVH Support, Birth to Career, and Santa Fe Community Foundation.
